Who is eligible?

Seniors may be eligible if they:

• Are 65 or older
 • Are a resident of Ontario
 • Are a single senior with annual net income of $25,000 or less, or senior couples (one or both people aged 65+) with a combined annual net income of $41,500 or less; &
 • Have no access to any other form of dental benefits including private insurance or through government programs such as ODSP, OW, or the Non-Insured Health Benefits.

What services are covered?

• Check-ups including scaling, fluoride & polishing
 • Repairing broken teeth & cavities
 • X-rays
 • Removing teeth or abnormal tissue (oral surgery)
 • Treating infection & pain (endodontic services)
 • Treating gum conditions & diseases (periodontal services)
 • Dental prosthetics (prosthodontic services), including dentures, are partially covered

Certain dental procedures under the OSDCP have limits based on scientific and clinical criteria; your OSDCP dental provider can provide more information based on your situation.

How to apply?

Seniors may get assistance completing the application.

Seniors will need to bring: 
 • A piece of ID 
 • Their Social Insurance Number (SIN) or Temporary Taxation number

If senior has a spouse (married or common-law), their spouse must also include their information on the form, including SIN or Temporary Taxation Number, & provide their signature for consent, regardless of their age.

If you wish to complete the application on your own, you can apply to the OSDCP online or through mail.

 • Complete the online application form
 • Print and sign the consent form
 • Mail the signed consent form within 30 days to:

Question 4

What if senior has no SIN or did not file recent taxes?

If you don’t have a SIN or didn’t file your taxes last year, a guarantor must help you apply by mail.(https://www.ontario.ca/page/dental-care-low-income-seniors#mail)

A guarantor is someone who can confirm the identity, age & residency of the individual or couple applying to the program.

The guarantor must be: 
 • A Canadian citizen, at least 18 years of age 
 • A member of a regulated profession (e.g., dentist, teacher, nurse, doctor, lawyer) or other specified profession (e.g., mayor, professor). Please visit ontario.ca/seniorsdental (www.ontario.ca/seniorsdental)for a list of acceptable guarantors
 • Able to confirm identity, age and residency of the individual applying to the program

Question 5

What if an applicant's spouse wants to apply?

Seniors need to apply for the program on an individual basis. If they have a spouse (married or common law partner) who would also like to apply for the OSDCP, they must complete their own application form.

What happens once a senior is approved?

Accepted Answer

Once an application has been approved, a welcome package and dental card will be mailed to the applicant.

To receive OSDCP services, the eligible senior must call the Niagara Falls Community Health Centre Dental Clinic 905-356-2288. The dental card must be presented at the time of appointment.

How are applications renewed?

The OSDCP benefit year runs from August 1 to July 31st.

Every year, eligibility will be automatically verified for most clients to confirm enrolment for the next benefit period.

You will be notified by mail if you are eligible to keep participating in the program, or about how to re-apply to the program, if required.

You must reapply if: 
 • you applied using a guarantor 
 • you did not file taxes for the most recent tax years that your eligibility is being determined
