We Are Accepting New Patients

If you live in the Niagara area and do not have a family doctor or nurse practitioner, you can fill out our intake package online today. At this time, all eligible applicants who complete the intake process will be accepted into care.

How to Apply

This intake form is for people who do not currently have a primary care provider, such as a family doctor or nurse practitioner. You do not need OHIP to apply.

How the Intake Process Works:

  • Step 1: Complete the online form
    Submit one intake form for each person applying.

  • Step 2: Our Intake Worker will contact you
    Our Intake Worker will call to confirm that you are still interested in becoming a patient at NFCHC.

  • Step 3: A nurse intake appointment will be booked
    If you are still interested, we will schedule a phone appointment with a nurse to review your medical history, current health concerns, and any supports you may need.

  • Step 4: You will be accepted into care
    Once your nurse intake appointment is complete, you will officially become a patient of NFCHC. You will then be rostered and able to book future medical appointments with your assigned primary care provider.

What to Expect as a Patient

Once you become a patient at NFCHC, you will have access to:

  • A primary care provider, such as a doctor or nurse practitioner

  • Mental health support

  • Dietitian services

  • Outreach and system navigation support

  • Occupational therapy

  • Kinesiology

  • Longer appointment times focused on your overall health and well-being

  • Help with referrals, community supports, and navigating systems

  • A welcoming, inclusive care environment
