Important info about vaccinations:
Please bring your yellow vaccination card to each appointment, and ensure you have an updated vaccine record prior to attending a Care for Kids Appointment.
To access your vaccine records online immediately or report any outstanding vaccines, please click here. Kindly check your yellow immunization card if you have one, as some primary doctors may not report for you. To connect with Niagara Region Public Health Vaccine Clinics to book an appointment if catch-up vaccines are needed, or if translation services are required to obtain an Ontario vaccine record in English, please click here.
Ensuring your records are current will enable our team to provide you with efficient and safe care during your appointment. If you need assistance updating your vaccine records, please call and make an appointment with our family support worker.
