How to Apply

What You Need to Know:

• One form per person. If other members of your household are applying, each person must complete their own form.

• Only for people without a healthcare provider. This intake is not for those already connected to a family doctor or nurse practitioner.

• Forms are reviewed in the order received. There is no guaranteed wait time.

• We’ll contact you when your name reaches the top of the queue, please don’t call to check your status.

• Once your intake is reviewed, you'll be added to our enrollment list. When it’s your turn, we’ll call you to book your first appointment.

• No OHIP required.

What to Expect as a Patient

When accepted, you’ll receive:

• A primary care provider (doctor or nurse practitioner)

• Access to our full team: mental health support, dietitian, outreach, occupational therapist, kinesiologist, & much more

• Longer appointment times focused on your overall well-being

• Help navigating systems, supports, and referrals

• A welcoming, inclusive care environment
