What is a Community Health Centre (CHC)?
Community Health Centres across Ontario, provide Health Care services that are delivered by a team of professionals consisting of a mix of Physicians, Nurse Practitioners, Registered Nurses, Health Promoters, Community Outreach Workers and other support professionals. As a Client of a Health Centre you will receive care and support from the whole Team. A change in staffing will not interrupt your health care since there are many qualified person to support you. When you make an appointment you will be booked with the medical professional who has the right skills to meet your needs. Quite often the skills of the Registered Nurse will be more than sufficient based on the nature of your need. In addition, the Nurse Practitioner has the ability to assess, diagnose, prescribe medications and order tests that are needed to keep you on the pathway to well being. The Physician is available to support you and the Team when the complexity of your needs require further skills and expertise. Always Remember, You Have a Team Supporting You!
Who funds the NFCHC?
We are funded by the Ministry of Health and Long-Term Care (MOHLTC)
Do I have to register?
Yes. In order to see one of health care professionals, use our lab or attend a program, you will enrol with the centre as a client. There is no fee to become a client.
How do I register?
Just come in and fill out the enrollment package. Then you will have an orientation with one of our Nurses to help decide who you should see and help you understand how the NFCHC team works together.
Can I use your services if I don't need a Doctor or Nurse Practitioner?
Absolutely! With the exception of our lab, our programs and services are open to residents of Niagara Falls, regardless of whether they visit a member of our medical team.
